=== Resistant gram negative bacteria === Investigators in the National Human Genome Research Institute and the NIH Clinical Center have used advanced DNA sequencing methods to characterize carbapenem-resistant enterobacteriaceae in hospitalized patients. They tracked a cluster of infections in hospitalized patients, then more recently, Frank's laboratory conducted a two-year follow-up study to understand the possible spread of the resistant genes between bacteria on plasmids. To further investigate the mechanisms used by bacteria to spread resistance, Frank's research program examined the frequency of horizontal gene transfer among species, using bacterial isolates from two hospitals. Most conjugation studies use a lab strain of E. coli, typically under narrow conditions. Given that conjugation is influenced by multiple factors, we sought to perform a systematic analysis of blaKPC encoding plasmids transfer into multiple species. The efficiency of conjugation into enterobacteriaceae patient isolates and a common lab cloning E. coli strain ranged widely from high rates of 10-2 or 10-3 CFU transconjugants/CFU recipients to undetectable, without a clear correlation with the pattern of spread suggested during two hospital outbreaks. In vitro models may not faithfully predict plasmid mobilization until we better understand the most important variables affecting conjugation efficiency.

Fluoxetine and norfluoxetine inhibit many isozymes of the cytochrome P450 system that are involved in drug metabolism. Both are potent inhibitors of CYP2D6 (which is also the chief enzyme responsible for their metabolism) and CYP2C19, and mild to moderate inhibitors of CYP2B6 and CYP2C9. In vivo, fluoxetine and norfluoxetine do not significantly affect the activity of CYP1A2 and CYP3A4. They also inhibit the activity of P-glycoprotein, a type of membrane transport protein that plays an important role in drug transport and metabolism and hence P-glycoprotein substrates, such as loperamide, may have their central effects potentiated. This extensive effect on the body's pathways for drug metabolism creates the potential for interactions with many commonly used drugs. Its use should also be avoided in those receiving other serotonergic drugs such as monoamine oxidase inhibitors, tricyclic antidepressants, methamphetamine, amphetamine, MDMA, triptans, buspirone, ginseng, dextromethorphan (DXM), linezolid, tramadol, serotonin–norepinephrine reuptake inhibitors (SNRIs), and other SSRIs due to the potential for serotonin syndrome to develop as a result. === Trump accounts and contribution pilot program === The law creates Trump accounts, a type of tax-advantaged savings investment account. Any individual is allowed to contribute to a child's account, up to $5,000 per year per child. Employers are allowed to contribute to their employees' accounts and their employees' children's accounts, up to $2,500 per year. Contributions by an employer count against the $5,000 annual limit per child, but contributions by the federal government do not. As an exception to the annual limit, tax-exempt organizations are allowed to contribute an unlimited amount into a child's account. Contributions into a child's account are allowed until the end of the end of the year in which the child turns 18. The federal government will contribute $1,000 into a Trump account for each U.S. citizen child with a social security number who was born between January 1, 2025, and December 31, 2028. Funds in the account must be invested in mutual funds or exchange-traded funds that mirror the S&P 500 or another U.S. stock index. Investment earnings are tax-deferred. A child with a qualified disability is allowed to rollover the funds into an ABLE account when they reach age 17. For other children, rollovers and withdrawals from the account are allowed starting on the January 1 of the year in which the child turns age 18. When the child reaches age 18, the funds will be rolled into a traditional IRA.
In December 2022, NBC News reported in a television segment that some TikTok and YouTube influencers were being given free and discounted cosmetic surgeries in order for them to advertise the surgeries to users of the platforms. In 2022, it was reported that a trend called "de-influencing" had become popular on the platform as a backlash to influencer marketing. TikTok creators participating in this trend made videos criticizing products promoted by influencers and asked their audiences not to buy products they did not need. However, some creators participating in the trend started promoting alternative products to their audiences and earning commission from sales made through their affiliate links in the same manner as the influencers they were originally criticizing. In June 2022, NBC News reported that some of the influencers paid by FeetFinder, a website that sells foot fetish content, did not disclose their videos were ads. FeetFinder said that it has suggested to influencers to be upfront about who was funding them. Existing sellers on FeetFinder said that the videos often misrepresented how "easy" it is to make money from posting feet pictures. Other TikTok creators have spoken out against accepting sponsorship deals indiscriminately and criticized those who posted undisclosed FeetFinder ads.
=== Transdermal and topical === A topical gel formulation of progesterone, for direct application to the breasts as a local therapy for breast disorders such as breast pain, was introduced under the brand name Progestogel in Europe by 1972. No transdermal formulations of progesterone for systemic use have been successfully marketed, in spite of efforts of pharmaceutical companies towards this goal. The low potency of transdermal progesterone has thus far precluded it as a possibility. Although no formulations of transdermal progesterone are approved for systemic use, transdermal progesterone is available in the form of creams and gels from custom compounding pharmacies in some countries, and is also available over-the-counter without a prescription in the United States. However, these preparations are unregulated and have not been adequately characterized, with low and unsubstantiated effectiveness.

In late 2022 the US Federal Motor Carrier Safety Administration denied a petition to recognize hair samples as an alternative (to the currently used urine samples) drug-testing method for truckers. The agency did not comment on the test validity, but rather stated, that it lacks the statutory authority to adopt new analytical methods. Although some lower courts may have accepted hair test evidence, there is no controlling judicial ruling in either the federal or any state system declaring any type of hair test as reliable. Hair testing is now recognized in both the UK and US judicial systems. There are guidelines for hair testing that have been published by the Society of Hair Testing (a private company in France) that specify the markers to be tested for and the cutoff concentrations that need to be tested. Conversely, this is not necessarily the case with oral or transdermal estradiol. Another major advantage of estradiol pellet implants is convenience and guaranteed compliance. They also do not have the issues pertaining to first-pass metabolism and liver protein synthesis of oral estradiol. A major disadvantage of estradiol pellet implants is that they cannot be easily removed should this be necessary. There are also concerns about accumulation of estradiol levels with long-term repeated pellet implantation. Estradiol levels may remain above baseline for a year or in some cases 3 to 4 years following the last pellet insertion. During this time, progestogen therapy should be continued to avoid the risk of endometrial changes. Regular monitoring of estradiol levels and adjustment of dosing is recommended during therapy with estradiol pellet implants. Tachyphylaxis of relief of vasomotor symptoms, or hot flashes returning even with normal or supraphysiological estradiol levels, may occur in a small subset of cases with estradiol pellet implants. The reason for this is unknown, but has been hypothesized to be a paradoxical effect of the high levels of estradiol achieved and/or a result of receptor desensitization caused by the long-term gradually decreasing levels of estradiol. Such symptoms have been said to occur once estradiol levels begin to decrease, although there are also reports of such symptoms occurring 3 to 16 weeks (1 to 4 months) after pellet insertion, when estradiol levels should still be constant.
