Clinical study of tesamorelin has centered on adults with HIV-associated lipodystrophy, a condition in which abdominal fat accumulates while peripheral fat is lost. In controlled trials, treated participants showed reductions in visceral adipose tissue measured by imaging, alongside modest shifts in some lipid values. Effects on subcutaneous fat were smaller and less consistent across studies. Whether these changes translate into fewer cardiovascular events remains an open question, because the trials were not designed or powered to answer it.
Tesamorelin is a synthetic peptide that acts as an analog of growth hormone-releasing hormone, a natural hypothalamic signal. Its sequence corresponds to the forty-four amino acid form of the human hormone, with a small acyl group attached near the amino terminus. That modification slows enzymatic breakdown and extends the time the peptide remains active in circulation. The compound was developed as a pharmacological way to raise endogenous growth hormone output rather than supplying the hormone directly.
After injection, the peptide binds receptors on somatotroph cells in the anterior pituitary. Receptor activation raises intracellular cyclic AMP and triggers release of stored growth hormone into the bloodstream. Because the compound works through the body's own regulatory system, growth hormone pulses retain much of their normal feedback control. Repeated administration also raises insulin-like growth factor 1, a hormone produced mainly in the liver. Investigators treat that rise as a marker that the pituitary axis has been engaged.
Clinical interest in tesamorelin arose from the need to address visceral adiposity in people living with HIV. Antiretroviral therapy improved survival but was associated in some patients with central fat accumulation, altered lipid profiles, and metabolic complications. This condition, often called HIV-associated lipodystrophy, involves excess visceral adipose tissue that is difficult to manage through diet and exercise alone. Investigators evaluated tesamorelin because GHRH analogs can stimulate growth hormone secretion and influence fat distribution without direct liposuction or invasive procedures.
A Phase 3 program led to regulatory approval in the United States in 2010 for reduction of excess visceral abdominal fat in adults with HIV and lipodystrophy. Subsequent studies examined effects on liver fat, muscle area, and metabolic markers, with mixed findings for some endpoints. Long-term cardiovascular outcomes and effects on mortality remain uncertain because most trials were relatively short and focused on imaging-based fat measurements. Use in populations without HIV has been studied experimentally but is not part of the approved indication.
Tesamorelin is a synthetic analog of growth hormone-releasing hormone, a peptide hormone produced by the hypothalamus. The molecule retains the 44-amino-acid sequence of human GHRH and carries a trans-3-hexenoyl modification at its N-terminus. This modification increases resistance to enzymatic degradation and extends the peptide's functional stability relative to native GHRH. The compound is supplied as a lyophilized powder for reconstitution and subcutaneous administration in clinical settings. Its development code was TH9507, and it belongs to the GHRH analog class. It is not a growth hormone product; instead, it acts upstream to stimulate endogenous growth hormone release.

== Function == While the whole hCG glycoprotein hormone is a heterodimer consisting of an alpha and a beta subunit, it is the beta subunit alone that is responsible for the specificity of function of hCG. Thus, in this section, we will be referring to hCG as the specific beta subunit CGB, due to the subunit being solely responsible for its relevant function in pregnancy. CGB is only produced by the human body during pregnancy. CGB is the very first specific molecule synthesized by the embryo, with its RNA transcribed as early as the eight-cell stage. It is the trophoblast, which is the outer layer of the blastocyst, that first synthesizes this glycoprotein hormone. The blastocyst will become the embryo, and the trophoblast will eventually become part of the placenta. The blastocyst/trophoblast release CGB into the uterine space, which is somehow able to reach the hCG/LH receptor on the endometrial surface. Again, it is specifically the beta subunit of hCG (and also LH) that binds to the receptor, as the alpha subunits of both hormones, as well as FSH and TSH, are the same. This nonvascular communication by CGB is highly remarkable, and it is still not fully understood how CGB is able to communicate with the endometrial surface without being attached to it. Binding of CGB to the hCG/LH receptor prepares the endometrium for the impending implantation of the blastocyst. CGB implements immunotolerance and angiogenesis at the endometrial maternal-fetal interface, which is particularly critical to the establishment of a successful pregnancy.

== Anaerobic == The most well-studied anaerobic FGE is the bacterial AtsB, an iron-sulfur cluster containing enzyme present in Klebsiella pneumoniae, that is able to convert either cysteine or serine to fGly with a distinctly different mechanism than the aerobic form. While AtsB can convert either, its activity increases four fold when in the presence of cysteine over serine. AtsB is 48% similar to an enzyme present in Clostridium perfringens. Both enzymes possess the Cx3Cx2C motif unique to the radical S-adenosyl methionine superfamily and are able to use a reduction reaction to cleave S-adenosyl methionine. These two enzymes fall into a larger group called the anaerobic Sulfatase Maturing Enzymes, which are able to convert cysteine into fGly without the use of oxygen.
== FFAR2-FFAR3 receptor heteromer == The FFAR2-FFAR3 protein dimer, also termed FFAR2-FFAR3 receptor heteromer, consists of single FFAR2 and FFAR3 proteins joined together. This dimer has been detected in monocytes isolated from human blood and macrophages that were differentiated from these monocytes (see monocyte differentiation into macrophages). Like other protein dimers, the FFAR2-FFAR3 protein dimer had activities that differed from each of its FFAR monomer proteins. However, FFAR2-FFAR3 dimers have not yet been associated with specific functions, clinical disorders, or clinical diseases.

=== Cardiovascular disease === A 2011 systematic review evaluated the effectiveness of creatine and creatine analogues in adults with cardiovascular disease, including heart failure and myocardial infarction. The studies assessed the use of various creatine-based compounds—such as creatine, creatine phosphate, and phosphocreatinine—administered via oral, intravenous, or intramuscular routes, typically as adjuncts to standard therapy. The analysis found no conclusive evidence that creatine or its analogues significantly affect mortality, myocardial infarction progression, or ejection fraction. However, some studies suggested a potential improvement in cardiac dysrhythmias and dyspnoea. The trials varied considerably in terms of drug formulation, dosage, treatment duration, and patient populations. Notably, no studies were identified that examined the effects of these compounds in patients with essential hypertension. Due to the small sample sizes, clinical heterogeneity, and inconsistent outcomes across trials, the authors concluded that more rigorous and larger-scale studies are necessary to establish the clinical utility of creatine analogues in cardiovascular care.

== History == Myxedema was first treated successfully in 1891 when George Redmayne Murray diagnosed a 46-year-old woman with the disease. He prescribed an extract from sheep thyroid. The patient improved significantly within a few weeks and lived another 28 years while taking the sheep thyroid extract.
Medical societies and professional guidelines recommend that the physician confirm a person is at high risk for Chronic Coronary Syndrome before conducting diagnostic non-invasive imaging tests to make a diagnosis, as such tests are unlikely to change management and result in increased costs. Patients who have a normal ECG and who are able to exercise, for example, most likely do not merit routine imaging.

It is a laboratory-made peptide that mimics growth hormone-releasing hormone. It prompts the pituitary gland to release growth hormone and has been studied mainly in adults with HIV-associated lipodystrophy.
Administered growth hormone supplies the hormone directly, while this peptide acts upstream by prompting the pituitary to release it. The indirect route preserves pulsatile secretion and some endogenous feedback, which changes the hormone and IGF-1 profile observed after treatment.
Reductions in visceral adipose tissue appear consistently in randomized trials of the approved population. Effects on peripheral fat, cardiovascular outcomes, and use outside that population are less well established.
It is a synthetic peptide analog of human growth hormone-releasing hormone. It is used clinically to reduce excess visceral abdominal fat in adults with HIV-associated lipodystrophy. It works by stimulating pituitary growth hormone release.
